HOME BASED CARE SOCIAL WORKER

Location: Johannesburg – Sandringham
Position Type: Fixed Term

Purpose of the Job

To promote, maintain, and develop the psychosocial functioning of beneficiaries and family support registered on the HBC programme within the community.  Plan and engage in group work and community programs. Work in close collaboration with the HBC team and the HBC Multi-disciplinary team.

Role and Responsibilities

Casework
• Counselling and case management of older persons and/or the beneficiaries within the community registered on the HBC Program.
• Advocate for clients’ rights, ensuring they receive appropriate services and support to maintain their independence and dignity.
• Complete and Individual Development and Treatment Plan for all beneficiaries.
• Complete needs assessments, evaluations and reviews for beneficiaries.
• Monitor and evaluate the effectiveness of care plans, making necessary adjustments as the client’s needs change.
• Report abuse and neglect according to the older persons act.
• Counselling and support services to the families of the older persons/ beneficiaries.
• Bereavement, grief counselling and related debriefing.
• Crisis Management, relevant follow up and monitoring as required.

Groupwork
• Group work and community programs.
• Collaborative working as part of a multi-disciplinary team.

Administration and other tasks
• Applying administrative processes as required.
• Completing relevant reports.
• Maintain accurate and up-to-date client records, documenting assessments, interventions, and progress.
• Holding case conferences / family meetings (collaborating with the multidisciplinary team as needed).
• Sharing of knowledge and supervising of volunteers and students to further professional development and upskilling.
• Attending group and/or individual consultation/ supervision with appointed consultants/ supervisors.
• Promoting active ageing programs.
• Maintaining records and statistics.
• Attending and participation in staff meetings.
• Attending multi-disciplinary team meetings.
• To ensure social services delivered according to care standards.
• Providing feedback to relevant teams consistently.
• Stay informed about current social work practices, policies, and regulations related to home- based care services.
• Maintain confidentiality and adhere to ethical standards in all interactions with clients and their families.

Qualifications and Education Requirements

• Relevant Social Worker Degree
• Registration with SACSSP
• Knowledge of Jewish culture and traditions would be beneficial
• Valid Driver’s License and Vehicle non negotiable
